TNVS CPC Requirements

TNVS Accreditation

• STEP 1.  Get Appointment Date for Case No. from LTFRB Online Registration

a. Pumunta sa LTFRB Appointment Scheduler: http://ltfrb.ph.net/tnvs

b. Kung may na-encounter kayong error sa pag accomplish ng inyong appointment online, please contact LTFRB directly.

c. Ilagay ang mga sumusunod na inpormasyon:

Operator

• Name
• Date of Birth
• Sex
• Address
• Email Address
• Mobile Number

 Vehicle information

• Plate/Conduction Number
• Vehicle Make (e.g. Toyota Vios)*
• Model Year (YYYY) - vehicle cannot be older than 3 years
• Chassis Number - makikita sa Certificate of Registration (CR) ng sasakyan mo
• Engine No./MV Number - nasa CR ng sasakyan mo

d. Pumili ng schedule sa kalendaryo at at click "Next"

e. I-check ang e-mail para sa "Registration Validation Code". Kopyahin at ilagay ito sa LTFRB Webpage.  

f. Makakatanggap ka sa e-mail ng "LTFRB Appointment Date Confirmation"

g. I-print at kumpletohin ang lahat ng mga dokumento at ipa-notaryo ito.

• STEP 2. Go to your Appointment Date and Submit Documents to get Case Number

Kailangang ilagay sa LONG ORANGE FOLDER ang lahat ng requirements at Application Form.

1. Apat na kopya (4) ng Notarized Verified Application  alleging proof of citizenship at financial capacity
2. 1 Photocopy of Proof of Filipino Citizenship (any of the following)
   • Authenticated Birth Certificate from Philippine Statistics Authority
   • Valid Philippine Passport
   • Voter’s I.D.
   • Marriage Certificate
3. 1 photocopy of Government Issued I.D. (my picture ng applicant)
4. 1 photocopy of LTO Official Receipt (OR) and Certificate of Registration (CR) of authorized unit/s na may year model na naka-pangalan sa applicant (ang hatchback ay hindi na pwede)
5. Operator’s Data Sheet (download here) na kumpleto ang detalye, latest picture at valid Driver’s License/s ng mga authorized driver/s.
6. Personal Appearance. The TNVS Applicant (ie. Vehicle Owner) is required to do  personal appearance on the day of the appointment date at the LTFRB Central Office, East Ave, Quezon City.

Fees for TNVS Application:

• Filing Fee – PhP 510.00 first 2 units
• Additional PhP 70.00 for 3rd unit
• LRF – PhP 10.00
• Inspection Fee – PhP 50.00
• PA Fee – PhP 250.00

• STEP 3. Get your PA from LTFRB

Matapos mag-file ng Application Form with Case Number sa LTFRB, kailangang bumalik sa LTFRB sa loob ng 30 working days para sa release ng PA at para alamin ang iyong hearing schedule.

Dalhin ang mga sumusunod sa pagkuha ng iyong Provisional Authority (PA):

• 1 Photocopy ng Verified Application Form (download here) with stamped Case Number at Official Receipt mula sa LTFRB
• Passenger Insurance (i.e. PAMI or SCCI) | How to get PAMI?
• Proof of Existence and Sufficiency of Garage
• Location map (Google Map)
• Dimension of Garage
• If the applicant is the owner of the garage: provide Transfer Certificate of Title (TCT)/Tax Declaration in the name of the applicant (vehicle owner)
• If the applicant leases the garage: Notarized Contract of Lease/Authority to use with copy of TCT of Lessor | How to get TCT?
• LGU Zoning Certificate for the location of the garage for three (3) units

Additional requirement for new applications who secured appointment date from July 29, 2019 onwards:

• Certificate of Conformity | How to get Certificate of Conformity? IMPORTANT NOTES:
• Ang PA ay valid sa loob ng 90 days, matapos ng 90 days at wala ka paring CPC, kailangang kumuha ng PA extension  hanggang mabigyan ka ng CPC. Siguraduhing kumuha ng PA Extension bago mag 90 days.
• Simula June 10, 2019, lahat ng NEW vehicle applications sa Grab ay kailangang may P.A. bago ma-activate sa platform; ayon ito sa bagong rules ng LTFRB. Wala itong epekto sa mga existing vehicles na activated na sa platform.
• Simula July 24, 2019 (Wednesday) – nagbukas ang LTFRB ng PA Appointment scheduler via link: ltfrb.ph.net/pa .

– Sino ang pwedeng kumuha ng PA Appointment?

• May Case No. pero wala pang PA
• Ang PA mo ay expired at nakapag-FILE ka na ng Motion for Extension
• Reminder: Ito ang format ng Case No. na dapat gamitin sa appointment: xx-xxxx (e.g. 12-12345).
• Nakakuha ng Interim Provisional Authority (IPA) stamp bago lumipas ang 30th working day mula nang makakuha mo ang iyong Case No, kailangan mo pa rin bumalik sa LTFRB sa 30th day upang alamin ang iyong hearing date.

• STEP 3.1 Secure PA Extension (if PA is expired)

• STEP 4. Attend CPC Hearing Date

Kailangan maghanda ng TNVS Applicant ng Formal Offer of Evidence (download FOE here) kung saan nakapaloob lahat ng requirements ng LTFRB bago ang scheduled CPC hearing.

Personal Appearance. Kinakailangan umattend ng TNVS Applicant (Vehicle Owner) sa CPC hearing at ipresenta ang mga original copies ng mga dokumento at requirements. Kung hindi available ang vehicle owner, maaaring magpadala ng representative na next of kin – direct descendant, spouse, o ascendant (magulang, asawa, o anak) ng vehicle owner basta may kaukulang SPA.

Kung hindi ka nakapunta sa iyong initial (1st) hearing date, siguraduhing pumunta sa LTFRB para alamin ang iyong reset hearing date schedule.

CPC Requirements:

• Proof of Existence and Sufficiency of Garage

• Certified True Copy of latest Income Tax Return received by the BIR (BIR Form 1701 o BIR Form 2316)
• Proof of Bank Deposit in the amount of Php 50,000/unit. Bank account should be under the name of the car owner (as indicated in the OR/CR)
• 5R Photograph of units subject of the application (front back and sides of the vehicles)
• 
  
• Note: Photo should include the front page of a newspaper to show the date when the pictures were taken.
• Notarized Affidavit of Applicant
• DTI Business Name Registration (National) | How to get DTI Business Name?
• BIR Certificate of Registration 2303 | How to get BIR 2303?
• Proof of Publication | How to get Proof of Publication?

1. Affidavit of Publication by the publisher
2. Clipping of newspaper
3. Must be secured 5 days prior the Hearing Date

• Certificate of Good Standing | How to request for Certificate of Good Standing from Grab?
• Passenger Insurance Policy (i.e. PAMI SCCI) | How to get PAMI?
• Authenticated Birth Certificate or Valid Passport
• Notarized Operator’s Data Sheet (download here) with complete details, latest 2×2 picture and a copy of valid Driver License/s of Authorized Driver/s.
• Certified True Copy of OR/CR

• Photocopy of Driver’s License ( for Driver )
• NBI Clearance ( for Driver )

• STEP 4.1 Dismissed Cases

Kung ikaw ay nakakuha ng ‘Dismissal Order’ pagkatapos ng tatlong (3) failed hearing dates, kailangan mong mag-file ng Motion for Reconsideration upang makakuha ng panibagong Notice of Hearing:

Dalhin ang mga sumusunod sa pag-file ng iyong Motion for Reconsideration:

• 4 copies of Motion for Reconsideration (prepared by a lawyer)
• Photocopy of OR/CR
• Photocopy of Dismissal Order
• Photocopy of Government ID (UMID, Police Clearance ID)
• Photocopy of Driver’s License
• Photocopy of Proof of Citizenship (Birth Certificate, Voter’s ID, Passport)
• Photocopy of DTI
• Original Operator’s Data Sheet w/ 2×2 attached
• Special Power of Attorney w/ 2 photocopy of government IDs (w/ 3 signature)

Important Note:
Sa mga dismissed cases, maaari ka lamang mag-extend ng iyong PA pagkatapos makakuha ng panibagong Notice of Hearing.

• STEP 5. Get your CPC from LTFRB

Congratulations, LTFRB Accredited ka na bilang isang TNVS.

IMPORTANT NOTES:
Starting 2018, ang CPC ay valid at renewable every 2 years.
Pwedeng magamit ang iyong sasakyan sa TNVS for 7 years (from date of manufacture).
